Just me? I often find myself saying sorry to teammates lately. It's mutual though. I get hit by teammates more and I'm hitting teammates more. edit: maybe due to the events? yeah that's prolly it
What I have noticed the most is PUGs cutting in front of a continuous stream of fire from an assault melting down a mech to take the kill shot. Then crying that you blew off parts or team killed them. Event brings that out more than usual.
Just now, at the start of the match, I accidentally hit the mouse and shot my teammate! I said sorry.

That is exactly what happened some minutes ago. Was rushing the LRM Boat, killing it and was then wondering why the hell is my back red. Not sure why everyone thinks that they can shoot through their own Assault...
I only have a problem when you get shot with no enemy around, someone testing their weapons or just not checking their fire. Otherwise friendly fire is just a thing that happens. I've had my share of moments, too. I've not seen that the mech in front of me is an overheated and shut down friendly, not the enemy just past him. Those make me feel bad. Others, when someone runs into my arty strike or between me and the enemy the moment I release the gauss, i don't feel bad about...that stuff happens.
